El Salvador expands AI telemedicine with DoctorSVCEFR B1

Since 2023 El Salvador’s government has framed artificial intelligence as central to rapid national progress and has made deals with major tech firms. Google opened an office in 2024, and the telemedicine app DoctorSV started in 2025 with backing from the Development Bank of Latin America and the Caribbean (CAF) and Google. In April 2026 authorities announced a second phase that integrates an AI assistant into the app.
Officials, including the president, described DoctorSV as a system that screens for chronic disease risks, creates lab orders from patient questionnaires, provides diagnostic classifications to doctors, and helps schedule and remind patients of follow-ups. Supportive outlets and a commentary in The Lancet presented the program as a model for resource-constrained settings.
Medical workers and unions have voiced concerns about deteriorating public healthcare, shortages, lack of transparency and the firing of more than 7,700 public healthcare workers in 2025. Cybersecurity experts also warn there is not enough information to guarantee protection of medical records.
