New research suggests the experimental drug Setanaxib can protect the heart in Duchenne muscular dystrophy (DMD). The study, published in Molecular Therapy, tested Setanaxib in two preclinical models of DMD and found that the treatment preserved cardiac function, limited heart enlargement and reduced tissue scarring. The results also highlight the enzyme NOX4 as a potential therapeutic target for cardiomyopathy, the serious form of heart disease often linked to DMD.
The study was led by Da-Zhi Wang, professor at the University of South Florida Health Morsani College of Medicine and director of the Center for Regenerative Medicine at the USF Health Heart Institute. The team included John Mably, associate professor, Gabriela Diniz, assistant professor, and outside collaborators. Wang noted that, despite progress in gene replacement therapies, DMD remains a devastating disease and that better understanding of its progression is urgently needed.
Setanaxib is designed to limit oxidative stress by targeting NOX1 and NOX4, enzymes that produce highly reactive molecules inside cells. At high levels these molecules cause inflammation and fibrosis. In the study Setanaxib reduced inflammation and fibrosis, lowered activity of genes associated with cardiomyopathy, and helped the heart keep its ability to pump blood. Mably noted that Nox4 inhibitors have already been tested in clinical trials for lung fibrosis and kidney and liver disease, and the team hopes for trials in DMD patients. Diniz said the work shows how basic biomedical science can help understand disease and improve care.
