Cryptococcal meningitis is a fungal infection that mainly affects people with HIV and causes severe illness. In 2014 two women, Elube and Patricia, arrived at Kamuzu Central Hospital in Lilongwe with worsening headaches; both became unconscious and were later diagnosed with HIV and cryptococcal meningitis.

Clinicians used lumbar punctures to reduce pressure inside the skull, which often eased severe headaches but did not always prevent long-term damage. Patricia lost her sight more than ten years ago and has not recovered it. Doctors explain that the fungus is common in the environment and can lie dormant until a weak immune system allows it to spread to the central nervous system.

WHO guidance changed from a week of amphotericin B deoxycholate plus oral flucytosine and fluconazole to a 2022 recommendation for a single high dose of liposomal amphotericin B with flucytosine and fluconazole. Because taking flucytosine four times daily is difficult, the Drugs for Neglected Diseases initiative is running a Phase II trial at Kamuzu using a sustained‑release pellet made by Viatris to reduce doses; the trial is halfway enrolled and results are expected in 2027. Clinicians say late presentation, weak diagnostics and low community awareness keep mortality high, and they call for more testing and earlier care.
