Researchers at the University of Copenhagen studied 130 people with severe obesity for one year after a low-calorie diet, during which participants lost an average of 13.7 kg (30.2 lbs). After the diet they were randomly assigned to one of four groups: exercise only, liraglutide only, exercise plus liraglutide, or placebo. The study measured vascular function and markers of inflammation to assess heart and blood vessel health after weight loss.
The key result was that the exercise groups showed improved vascular health and lower inflammation. Specifically, carotid artery wall thickness — a marker of cardiovascular risk — fell by about 6–7% in those who exercised. The liraglutide-only and placebo groups did not show vascular improvement. Combining exercise with medication produced further weight loss and improvements in several vascular markers, while medication alone helped maintain weight loss but did not improve vascular health.
Participants trained on average roughly two and a half hours per week, mainly spinning and circuit training, and activity was measured objectively with heart-rate monitors. The volunteers were in their mid-40s and had not yet developed diabetes or heart disease. The study examined liraglutide, an earlier-generation GLP-1 weight-loss drug; the authors note that the results cannot be directly applied to newer drugs such as semaglutide (for example, Wegovy).
