Researchers report that struggling to read more than six lines on an eye chart with fading letters may be an early warning of vision problems that routine exams sometimes miss. Traditional charts use high contrast—black letters on white—and measure visual acuity, but they can miss changes that affect daily life. Contrast sensitivity testing presents letters in progressively lighter shades of gray and can reveal trouble recognizing faces, reading print, or watching television. Poor contrast sensitivity has also been linked to falls, difficulty driving, and loss of independence.
The investigators used data from 4,475 Medicare beneficiaries in the National Health and Aging Trends Study, making the results nationally representative. The authors note that labeling scores two standard deviations below the mean only shows someone is worse than average; it does not indicate whether the person will struggle in daily life.
